In addition, in order to maximize your WorldPerks membership, from time to time you may receive notification of exclusive discount fares, special offers and select promotions from Northwest and its partners via mail or e-mail sent from Northwest only.

    このボーナスマイル加算申請フォームによるご意見、ご要望はお受けできませんので、余白にはご記入にならないようご注意ください。搭乗券を発行しないタイプのチェックインの場合は、チェックイン時に各航空会社から発行されるもの（搭乗者名、搭乗便名、搭乗日、座席番号、区間が明記されている「お客様控え」など）でも代用が可能です。チェックイン時、もしくは搭乗ゲートにて、搭乗券・搭乗半券が発行される場合は、搭乗者名、搭乗便名、搭乗日、座席番号、区間が明記されていても「ご利用案内書」、「引換証ご利用案内書」、「搭乗案内書」、「搭乗証明書」などは、ボーナスマイルの加算対象外となります。必ず搭乗券・搭乗半券を添付してください。
    This MILEAGE CREDIT REQUEST FORM cannot be used for change of address or other general WorldPerks inquiries. If you check in without a boarding pass, you can use another proof of check-in issued by the airline to claim your WorldPerks Bonus Miles (e.g. Receipt with Passenger Name, Flight Number, Departure Date and Seat Number and Destination). For flights the boarding passes are issued at check-in or at the gate, notices such as Ticket Issuance Notice, Boarding Notice, Boarding Certificate are not eligible for bonus miles. For this case, please attach the copies of boarding passes (stubs).
