Use the form below to request credit to your account. Retroactive credit will not be given for flights taken prior to enrollment into the SkyMiles® program. You must submit your mileage credit request within nine months of the date of your flight or a claimed discrepancy cannot be considered for credit.

Please provide your ticket number and SkyMiles (or former WorldPerks) account number. Your 13-digit ticket number can be found on your E-Ticket Trip Summary Receipt or ticket. If Northwest is unable to locate your ticket information, you will be prompted to download the mileage request form, then return the completed form to the nearest SkyMiles Service Center together with a copy of your original boarding passes and a copy of the ticket (or E-Ticket receipt).

All requests for mileage credit are subject to verification and approval by Northwest Airlines.
